چکیده

Ten patients with valvular pulmonic stenosis were studied by simultaneous external phonocardiograms and intracardiac pressure recordings during successive respiratory cycles to examine the mechanism of the respiratory variation in the ejection click. Selective cineangiograms were perforned in four. During inspiration right ventricular end-diastolic pressure (RVEDP) exceeded the pressure in the pulmonary artery (+ 2.7 mm) and no ejection click was recorded. During expiration RVEDP was lower than the pressure in the pulmonary artery (-3.6 mm) and a click was recorded. The click was present throughout the respiratory cycle when pulmonary artery diastolic pressure consistently exceeded RVEDP. Clicks were associated with sudden "doming" of the valve demonstated by cineangiography. These data support the valvular origin of the ejection click in pulmonic stenosis and provide the following explanation for the respiratory variation: Inspiratory increase in venous return causes the valve leaflets to move to an

Heart murmurs are a common finding during physical examination of children but most of them are innocent or normal.recognizing the etiology of heart murmurs can help us in differential diagnosis of them.the peresent study was undertaken to determine the prevalence and etiology of heart murmurs  in 4761 school children.the sex ratio was nearly 1:1 ,murmurs were heard in 459 students (9.64%) with...

Successful treatment of a congenital pulmonic valvular stenosis in a snow leopard (Uncia uncia) by percutaneous balloon valvuloplasty.

A 3-yr-old intact female snow leopard (Uncia uncia) was evaluated for progressive apathy, lethargy, and decreased appetite. Cardiac auscultation revealed a left basal grade IV/VI systolic ejection murmur, and an echocardiogram confirmed a severe pulmonic valvular stenosis (pressure gradient of 98 mm Hg). The lesion was managed by balloon valvuloplasty, resulting in a marked pressure gradient re...

A Hemodynamic Technic for the Detection of Hypertrophic Subaortic Stenosis

IT IS NOW well appreciated that following a successful valvulotomy for valvular pulmonic stenosis, muscular hypertrophy in the right ventricular outflow tract may persist and be responsible for a significant resistance to right ventricular ejection.'-3 Similar muscular obstruction to the left ventricular outflow has also been noted in patients following surgical relief of discrete valvular or s...

Balloon dilation of congenital supravalvular pulmonic stenosis in a dog

Percutaneous balloon valvuloplasty is considered the standard of care for treatment of valvular pulmonic stenosis, a common congenital defect in dogs. Supravalvular pulmonic stenosis is a rare form of pulmonic stenosis in dogs and standard treatment has not been established. Although, there have been reports of successful treatment of supravalvular pulmonic stenosis with surgical and stenting t...

Body surface potential maps in patients with pulmonic valvular and aortic valvular stenosis of mild to moderate severity.

Multiple-lead surface potential maps were compared throughout ventricular activation in nine patients with mild-to-moderate pulmonic stenosis and in 12 patients with mild-to-moderate aortic stenosis. Abnormal patterns of potential distribution were found in aortic stenosis, including three patients without electrocardiographic evidence of left ventricular enlargement. When related to the onset ...
